Enrollment Process

Step 1 - Gather Your Documents

Copies of the following documents are required:

  • A completed Student Data Collection Form
  • A birth certificate or baptismal certificate
  • Up-to-date immunization records
  • Proof of address (drivers license or utility bill)
  • High school Transcripts/Proof of Promotion
  • Proof of custody if Parent/Guardian are divorced
  • Testing records (if available)
  • Withdrawal Form

Step 2 - Complete the Student Data Collection Form

Download, print, and fill out the Student Data Collection Form (PDF format).
Bring the completed form with you when you come for an interview.

Step 3 - Schedule an Interview
